Réponse Immunitaire chez les Malades Porteurs d’un Sarcome de Kaposi à l’Hôpital Général de Yaoundé

RESUME OBJECTIF: Decrire certains marqueurs de la reponse immunitaire au cours du sarcome de Kaposi (SK). MATERIEL ET METHODES : Etude descriptive dans le Service d’Oncologie Medicale de l’Hopital General de Yaounde, constituee de patients presentant un SK et naifs de tout traitement anticancereux. Les taux sanguins des LTCD4+, IgG anti-HHV8, et des cytokines inflammatoires IL-6 et IL-10, ont ete mesures et leurs valeurs correlees avec les donnees demographiques et cliniques des patients. RESULTATS : Sur les 52 patients retenus, 61.5% etaient de sexe masculin. La moyenne d’âge etait de 42.27±11.48 ans. L’association du SK a l’infection a VIH a ete retrouvee, divisant notre echantillon en deux groupes : celui presentant le SK epidemique (SK+VIH) a 90.4%, et celui avec un SK endemique a 9.6%. 50% d’atteinte exclusive des membres inferieurs a ete observee, avec les macules comme lesion cutanee majoritaire. La recherche en IgG a ete positive chez 92.3% des patients, avec une valeur mediane de 3.12UI. Le taux median de LTCD4+ a ete de 170cellules/mm3, avec 79% des patients immunodeprimes. Seul le SK endemique a eu des patients immunocompetents. La valeur mediane des taux d’IL-6 a ete de 2.05g/ml, et celle d’IL-10 de 3.08g/ml, avec respectivement 54 et 56% des patients presentant des valeurs anormales. Les valeurs anormales d’Il-6 ont ete associees a l’âge avance, a la presence de l’infection a VIH et a la prise des ARV. CONCLUSION : La reponse immunitaire au cours du SK, au sein de notre echantillon, se traduit par la presence des taux anormaux des cytokines IL-6 et IL-10, la presence des IgG anti-HHV8 et les taux bas de LTCD4+. ABSTRACT AIM: To assess certain immune response markers during Kaposi sarcoma (KS). MATERIAL AND METHODS: A descriptive study was carried out at the medical oncology unit of the Yaounde General Hospital, on patients with clinical manifestations of KS and not taking any cancer treatment. A serological test was performed on these patients to measure their LTCD4+, anti-HHV8 IgG, and inflammatory cytokines IL-6 and IL-10. The results were then compared to their demographic and clinical findings. RESULTS: Out of the sample space of 52 patients on which we were working, 61.5% were male with an average age of 42.27±11.48 years. We further divided the sample space into two groups: those who had KS in association with HIV also called epidemic KS and those who had just KS also called endemic KS, found respectively in 90.4% and 9.6%. 50% of the population was infected exclusively on the lower limbs with the macula as majority skin lesions. The results of the IgG test carried out proved positive on 92.3% of the participants with a median of 3.12UI and that of LTCD4+ was 170cells/mm3 with 79% of the participants being immunodeficient. Only endemic KS presented non-immunodeficient patients. The median value of IL-6 was 2.05g/ml, while that of IL-10 was 3.08g/ml, with 54 and 56% of participant’s respectively presenting abnormal values. Abnormal values in IL-6 were also observed in participants with HIV infection who were under ARV and were also advanced in age. CONCLUSION: Based on the study carried out on our sample population, we can say that, immune response during KS can be described as follows: abnormal levels of IL-6 and IL-10 cytokines, low levels of LTCD4+ and the presence of anti-HHV8 IgG.
